Reuters - Los Angeles has earned the dubious distinction for the second year in a row as the U.S. city with the most dog attacks on mail carriers, the U.S. Postal Service said. Californias biggest city accounted for 69 such attacks in fiscal 2012, which ended in September, compared with 42 attacks each in Seattle, Washington, and San Antonio, Texas, which tied for second place. Chicago ranked third with 41 attacks.

Insurance Journal - Michigans attorney general has charged the former CEO and a bookkeeper with embezzling millions from a workers compensation insurance-related organization. Michigan Attorney General Bill Schuette announced felony charges for Jerry Stage, 67, of New Hudson, and George Bauer, 55, of Commerce Township. The charges allege that Stage, former CEO of the Compensation Advisory Organization of Michigan (CAOM), embezzled approximately $2.6 million from the company during an 11-year period

Reuters - An Airgas Inc distribution center in Putnam Country, West Virginia, has been closed after two people were injured in an explosion and fire at the facility, the company said on Monday. The center handles industrial gases including nitrogen, acetylene and oxygen that do not pose an immediate threat to public safety, though a section of adjoining highway has been closed, said Jason Owens, deputy director of public safety at the Putnam County Office of Emergency Management.

Around 10:00 Saturday morning on Harley Street, officials say 53-year-old Kenneth Drayton of Hollywood fell into a hole at a construction site. "Initial investigation indicates the victim was on a backhoe moving dirt when he somehow fell into the hole and the dirt gave way covering the victim. A co-worker noticed the victim was missing and began to search for him when he noticed the hole covered with dirt.The victim was located beneath the dirt deceased," North Charleston Police spokesman Spencer Pryor said.

Reuters - An oil field worker was killed and two others injured in a fire near a production well in Utah owned by the Newfield Exploration Company, a company official said Wednesday. The fire broke out in equipment near the well in Utahs Uintah County at 8:30 p.m. local time on Tuesday while two contract workers and one Newfield employee were conducting routine maintenance, the company said. The fire at the well site, about 130 miles east of Salt Lake City, was put out by local Utah crews Tuesday night.

CNN - A referee for a recreational soccer league who was punched in the face by a teenager has died, police in Salt Lake City said. Ricardo Portillo, 46, was refereeing a game in the Salt Lake City suburb of Taylorsville on April 27 when he cited a player for an infraction and issued him a "yellow card." In soccer, a yellow card is a caution to the player; a second results in his ejection from the game.

kgw.com - The man killed at a Clackamas meat distribution plant after falling into a large meat blender Saturday has been identified as 41-year-old Hugo Avalos Chanon, of Portland, police said. Deputies were called just before midnight to the report of a death at Interstate Meat Distributors, located at 9550 SE Last Road, said Deputy Marcus Mendoza of the Clackamas County Sheriffs Office. Several workers from DCS Sanitation Management were cleaning the facility and equipment when Chanon apparently fell into a meat blender that was in operation, Mendoza said.

The accident happened just before 10:30 a.m. at the Jacaranda Beach Villas, which are resort condominiums at the TradeWinds. Mark Allen Johnson, 45, was performing routine maintenance with a co-worker on the ground floor of the south elevator facing the beach when authorities said it appears as though the elevator dropped on top of him.
