Standard 4-page motor vehicle collision report for the state of California details all circumstances of an accident. Codes are used to represent roadway conditions, collision type, traffic control devices, safety equipment, contributing factors and pre-collision actions.
The Nevada motor vehicle accident report form identifies persons, vehicles and circumstances involved. It includes a crash narrative, non-motorist information, an occupant/witness supplement, and accident report codes.
The traffic crash report form for Illinois includes driver and vehicle identification, tow and insurance information, and description of property damage and bodily injuries. An accident diagram and narrative are included.
Memorandum for Appraisal is used in property damage situations where insured and insurer disagree on actual cash value, and is binding on the parties as to the amount of loss.
A Long Form report (HSMV 90010S) in its entirety must include a Narrative/Diagram when the following criteria are met:
- Resulted in death of, personal injury to, or any indication of complaints of pain or discomfort by any of the
parties or passengers involved in the crash;
- Leaving the scene involving damage to attended vehicles or property (F.S. 316.061(1));
- Driving while under the influence (F.S. 316.193);
- Rendered a vehicle inoperable to a degree that required a wrecker to remove it from the scene of the crash; or
- Involved a commercial motor vehicle. The Short Form (HSMV 90010S) designation is used to report other types of traffic crashes.
