A Long Form report (HSMV 90010S) in its entirety must include a Narrative/Diagram when the following criteria are met:
- Resulted in death of, personal injury to, or any indication of complaints of pain or discomfort by any of the
parties or passengers involved in the crash;
- Leaving the scene involving damage to attended vehicles or property (F.S. 316.061(1));
- Driving while under the influence (F.S. 316.193);
- Rendered a vehicle inoperable to a degree that required a wrecker to remove it from the scene of the crash; or
- Involved a commercial motor vehicle. The Short Form (HSMV 90010S) designation is used to report other types of traffic crashes.
The New Mexico crash report form describes persons, vehicles and circumstances involved, and includes contributing factors and a crash diagram/narrative.
The standard traffic accident report used in the state of Georgia, which includes driver, passenger and vehicle identification, and an accident diagram.
This report is completed by the driver responsible for bodily injury, death or property damage resulting from a motor vehicle accident in Illinois. It includes driver and vehicle identification, diagrams to indicate vehicle damage, tow and insurance information, and description of property damage and bodily injuries. An accident diagram and narrative are required to be included.
The South Dakota motor vehicle traffic accident report for truck/bus/hazmat describes the persons, vehicles and circumstances involved, and contains an accident diagram and narrative.
The North Carolina collision report form describes persons, vehicles and environment involved, including contributing circumstances, crash sequence, crash diagram and narrative.
This report must be filed by an owner or driver involved in a crash involving death or injury, or in which damage to property was in excess of four hundred dollars ($400) to any person involved. This report is required regardless of who was at fault and in addition to any report filed by an
investigating officer.
