This report is completed by the driver responsible for bodily injury, death or property damage resulting from a motor vehicle accident in Illinois. It includes driver and vehicle identification, diagrams to indicate vehicle damage, tow and insurance information, and description of property damage and bodily injuries. An accident diagram and narrative are required to be included.
